چکیده

This article reviews the use of intraoperative echocardiography during repair of congenital heart defects. Although initial experience was generated using epicardial transducers, there has been a trend in recent years toward the use of transesophageal echocardiography (TEE) in the operating room. This has encouraged increased involvement from cardiologists and anesthesiologists. New probe designs have provided biplane imaging via the TEE approach in infants weighing more than 2.5 kg. Smaller infants may still require epicardial imaging, so it is helpful for surgeons to maintain some skill in this technique. This article reviews the utility of intraoperative echocardiography for various congenital heart defects by providing examples from our experience at Duke University Medical Center since 1987 with close to 2,000 cases. Furthermore, we review and report for the first time our experience with TEE since 1993 in the operating room during infant heart surgery (493 patients). Along with this experience, we provide a review of important series in the literature to outline recommendations for the use of echocardiography during infant heart repair.

Transesophageal echocardiography for congenital heart disease.

Surface echocardiographic imaging of small children is routinely successful in defining anatomical details and Doppler flow patterns with even the most complex congenital cardiac malformations. However, in larger children or adults, imaging is frequently limited. A recent expansion of the role of echocardiography is intraoperative epicardial imaging. Epicardial and postoperative imaging, how...

Intraoperative transesophageal echocardiography during surgery for congenital heart defects.

OBJECTIVE This study was undertaken to further define the impact of intraoperative transesophageal echocardiography during surgery for congenital heart disease and to determine appropriate indications. METHODS The impact of transesophageal echocardiography on patient care was assessed in 1002 patients who underwent this procedure during surgery for congenital heart defects. It had major impac...

Educational program for intraoperative transesophageal echocardiography.

A lthough both disciplines have cooperated for many years, cardiovascular anesthesiology and echocardiography have recently grown and merged in part. Intraoperative echocardiography has demonstrated its usefulness in many aspects of cardiovascular surgery, including valvular repair and replacement, myocardial function, aortic aneurysm repair, congenital heart disease, pericardial disease, compl...

Costs and cost-effectiveness of routine transesophageal echocardiography in congenital heart surgery.

BACKGROUND The safety and efficacy of transesophageal echocardiography (TEE) during congenital heart surgery is well established. The economic costs and benefits associated with its routine use in this setting are, however, uncertain. We sought to analyze the impact that routine intraoperative TEE had on echocardiographic costs in the setting of congenital heart surgery. METHODS AND RESULTS A...

Changes in surgical conduct due to the results of intraoperative transesophageal echocardiography.

BACKGROUND AND OBJECTIVES Transesophageal echocardiography (TEE) is extremely useful in surgeries like valvuloplasty, of the thoracic aorta, and correction of congenital cardiopathies. The low degree of invasiveness and the capacity to aggregate information that can change the course of the surgery are among the advantages of TEE. The objective of this report was to present a case in which the ...
